Since 2019, ConcertoCare has used predictive analytics to help identify high-risk populations and those who with rising risk factors to provide in-home, individualized care. As a risk-bearing provider of field-based complex health services, ConcertoCare partners with providers, payers and the Center for Medicare and Medicaid Services (CMS) to provide a targeted care plan.
